Membership clubs are not insurance. They are payment arrangements for services rendered at Pearl Shine Dental. Memberships are available exclusively to uninsured patients and should not be considered pre-payment for future services. Patients agree to make yearly subscription payments for preventive care in place of paying at each visit.
Payment for cosmetic or restorative treatments is due at the time of service. Membership dues must be current to receive benefits, and discounts cannot be combined with other offers. It is the patient's responsibility to schedule and keep all appointments. Refunds will not be issued for dues paid, including missed or canceled visits.
